Abstract

Lupus nephritis (LN) is a severe renal complication of systemic lupus erythematosus (SLE) that presents with a broad spectrum of clinical and histopathological manifestations. While classical presentations include proteinuria, hematuria, and renal dysfunction, atypical cases can mimic other glomerular diseases, leading to diagnostic challenges. This case report describes a rare manifestation of LN in a young female presenting with nephrotic-range proteinuria and minimal immune complex deposition on renal biopsy. Clinical images, including histopathology and immunofluorescence findings, are provided to illustrate the diagnostic complexity. The report highlights the importance of a multidisciplinary approach in evaluating unusual LN presentations and emphasizes the role of repeat biopsy and advanced serological markers in challenging cases.
